General
Yes, SCOs will be required to complete data submissions each year as a part of Performance-Based Contracting. PBC data collection occurs during the months of February and March each year.
For the first year of PBC, ODP provided the baseline data for measures the SCOs did not have access to. Going forward, each SCO’s baseline data will be gathered from the previous calendar year, unless otherwise noted within the measure guidance.
The measures will progress with the goal of improving services for individuals and families. Small changes will be made from year to year. Larger changes will happen every 3 years.

As of January 1, 2026, the Associate SC position can bill under the 15-minute unit Fee-for-Service. Examples of the tasks an Associate SC could do, but are not limited to, are:
– Referrals to community resources (outside of ODP-funded services)
– Returning phone calls to individuals and families
– Preparing documentation/applications on behalf of another person
– Performing general updates to the ISP
– Facilitating the use of Charting the LifeCourse (CtLC) tools with individuals and families (Note: To facilitate LifeCourse tool(s), an Associate SC must have at least one year of experience working with the ID/A population and must have successfully completed both: Introduction to Charting the LifeCourse Framework (required during SC Orientation) & Charting the LifeCourse (CtLC) Person-Centered Fundamentals.
SCOs should work with their AEs to determine when an individual meets criteria for intensive TSM. An individual meets criteria for intensive TSM when they have active Medical Assistance, are not enrolled in a waiver, and their support needs are significant enough that the SCO is consistently performing activities listed in Billing Guidance List A or B for Intensive TSM for at least 3 months in a 6-month window.

SCOs should submit claims for services provided to individuals once they meet the minimum billing requirement as outlined in the Supports Coordination Minimum Activity for Billing document. SCOs will not need to submit documentation in advance of billing, however SCOs must maintain any and all documentation to substantiate claims. ODP will continue to complete claims documentation reviews.
Effective January 1, 2026, when ODP determines that there is a need for additional Supports Coordination Organizations, new applicants will be invited to qualify for enrollment through a competitive request for application process.
MyPBC Portal
MyPBC Portal will open to receive PBC submission forms from mid-February to mid-March annually. Submissions will only be received during this window.
SCOs that are part of larger organizations should only include the staff performing activities subject to the ODP Agreement for Supports Coordination Services.
The average time to complete a PBC submission varies by SCO but typically takes 2 hours.
No, users do not have to complete an entire submission in one sitting. MyPBC Portal allows users to enter data, save and exit the system, then log in at a later time to finish. A user may return to the submission as often as needed until the deadline for the submission window closes.
Yes. MyPBC Portal does not allow users to complete a submission without answering all required questions. If a user does not complete a mandatory question (indicated by a red asterisk) or navigates away from a page without saving, they will be prompted with a red error message or confirmation pop-up. Some questions have rules on what types of characters users can enter or file upload requirements. Users will see a red error message if they enter a response that does not meet requirements.
Data on the My PBC Profile page is sourced directly from other ODP systems (e.g., HCSIS). If your SCO disagrees with the outcome of an ODP data measure, you may use the PBC Resolution Process to submit information that supports your position. Please review the FAQ titled “What is the PBC Resolution Process?” for further information.
It depends. SCOs are highly encouraged to review measures and process details year after year, to identify any modifications made to an existing measure. If your SCO’s submission passed in the previous PBC review period, and the measure remains unchanged, it is probably appropriate to resubmit the same document.
However, many PBC measures will be modified year after year as part of continuous quality improvement (CQI), so careful review is suggested when measures are released for the next PBC review period to ensure you are prepared to meet them.
All previously unmet measures should be reviewed to identify potential process improvements, and the corresponding documentation should be updated as necessary to ensure compliance in the next PBC review period.
PBC submissions that are successfully received by ODP will have a status of “Completed” on the My PBC Profile screen within the MyPBC Portal.
SCOs should navigate to the My PBC Profile screen to review the column titled “PBC Submission Status” and confirm it shows a status of “Completed.” Designated MyPBC Portal users will also receive a confirmation email after ODP receives the completed PBC submission.
After ODP receives the submission, the information will be reviewed and ODP determines the measures the SCO has met. The SCO will receive written notification of their PBC determination once all determinations have been made.
The PBC Resolution Process is an optional activity that occurs after PBC determination letters have been distributed by ODP.
This process allows SCOs who believe the Department made an error in their PBC determination an opportunity to submit information to ODP supporting their position.
This is not an opportunity to correct an unmet measure, rather an opportunity for the SCO to submit information that supports their position that the Department made an error when determining if a measure was met.
Upon receipt of the SCO’s materials, the Department will reexamine its findings, and the SCO will be notified in writing regarding the updated determination, including measure(s) evaluations that were changed or remained the same based on the information submitted.
Performance Measures: Access
ODP has not established specific caseload ratio requirements for Supports Coordinators. Instead, the expectation is that each SCO demonstrates its ability to consistently meet the activity and quality standards for the individuals they support.
A referral is the process by which the SCO’s services are sought by an individual. This includes newly registered individuals to services and individuals seeking transfer to a new SCO.
No. This measure is calculated at the SCO MPI level, therefore the average referral acceptance rate for all the SCO service locations must be at 75%. If an SCO serves less than 250 individuals, the acceptance rate is 60%.
Performance Measures: Administration
Yes, the county’s audited Financial Report will meet the measure criteria. It is not necessary to have a separate audit or financial statement for the SCO services.
For county-based SCOs, measures ADM.01.3 and ADM.01.4 will not be applicable. County-based SCOs will indicate that they are county-based within the PBC Submission Form and will be exempt from answering these measures.
Performance Measures: Continuum of Services
This lead is someone with expertise and knowledge of housing resources in the areas served by the SCO; including subsidies, HUD, etc. This is separate from the provision of the Housing Transition and Tenancy Sustaining service.
Unlicensed Residential Habilitation services fall under the Residential Habilitation service as a provider-owned and operated setting.
Performance Measures: Person-Centered Practices
The measure calculation is based on validating the following timeframes: P/FDS – every 3 months; Consolidated & Community Living – every 2 months. This allows flexibility for the monitoring to be completed at any point during the month it is due.
Performance Measures: Quality (Data Integrity)
ODP will share the data used for these measures and the evaluation outcomes for these measures with SCOs on the MyPBC Profile screen. The data are pulled from HCSIS, CIS, EIM, PROMISeTM, IM4Q, OVR, CWIS, and PA Navigate (FindHelp).
“Intent to enroll” is a status within HCSIS Waiver Program Enrollment. This represents the date the intent to enroll letter is received and the individual is placed in the waiver queue. Individuals with this status will not be included in the calculation.
Performance Measures: Quality
All surveys will be administered by ODP. SCOs should plan to support this administration by encouraging family members to complete them.
The SCO’s QM Policy should include procedures for how the SCO approaches QM within the organization. This policy should drive the quality of work, clearly communicate to staff and regulators, and ensure that everyone is on the same page. The policy should clearly outline the process of how the SCO develops, updates, and maintains their QM Plan, and who is responsible.
SCOs should not upload the QM Plan itself. Please refer to the process details, which specifically identify the six (6) elements ODP expects to be included within each SCOs policy.
ODP QM certification lasts for two (2) years. Recertification materials are made available on the MyODP website for those who need to recertify, in the beginning of the calendar year in which recertification is due. Recertification is also good for two (2) years.
Yes. Within the measure question, there is a dropdown option of “In Progress” your SCO can choose.
Performance Measures: Risk Management
Trend analysis is the process of examining data over time to identify patterns, changes, or consistent movements. It helps you understand whether something is increasing, decreasing, or remaining stable, and can be used to make predictions or guide decisions (responses) based on what the data and analysis are telling you. Performance Measures: Resource Navigation
SCOs must first search for your program on the following website: https://findhelp.org/claims. If your program is not found, follow the instructions to “Suggest a Program” and enter your SCO program information.
Performance Measures: Workforce
SCOs should report the average for each area for the position, i.e. the average the SCO pays in PTO for the SCO position, the average the SCO pays in healthcare costs for the SC Supervisor position, the average the SCO pays in overtime for each of those positions, etc.
Financial/Billing
HCSIS service notes are being updated to capture both List A and List B activities to assist SCOs with tracking. The Service Note Report extract will also be updated to reflect changes in service notes relevant to List A and List B activities. Changes to support tracking of List A and Be activities will be available in HCSIS January 1, 2027.
The monthly rate assumes that SCOs are completing all necessary tasks for the provision of supports coordination. Lists A and B are those triggers for payment.
Yes, service notes are a frequently relied upon source of information for supports coordination, AEs, and ODP. Service notes contain key details about discussions, decisions, identification of issues and more. ODP requests that SCs continue to write comprehensive service notes.
SCOs are advised to have all claims for SFY 26-27 submitted by September 30, 2027. Beginning October 1st, ODP will conduct a comparison of 25-26 revenue to 26-27 revenue by SCO. For qualifying SCOs, stabilization payments will be made November – December 2027. SCOs subject to payment adjustment will be notified in January with instructions for cost report submission as applicable.
Pay for Performance Initiatives
If an SCO’s submission is determined to meet P4P criteria, the organization will receive an incentive payment valued at the lesser of their P4P budget request for the initiative or 3% of their total ODP-eligible revenue in the previous fiscal year. This excludes base and AAW revenue. For example, if the SCO collected $1M in claims revenue during this period and requested $50K via their budget, they would receive $30K (3% of $1M).
The available amounts are published in the Supports Coordination Services Manual. P4P Determination Letters will be distributed to SCOs via email once submissions have been reviewed and determinations are made.
If an SCO disagrees with ODP’s determination, the SCO should initiate the PBC Resolution Process via the MyPBC Portal.
For the capacity building P4P, SCOs should keep standard documentation such as invoices, proof of payment, training records, etc.
