Independent Monitoring For Quality (IM4Q)

Independent Monitoring for Quality (IM4Q) monitors the satisfaction and quality of life of people receiving services in the developmental disability system. IM4Q indicators are organized into the areas of individual satisfaction, dignity and respect, choice and control, inclusion, and physical setting.

IM4Q teams develop considerations from face-to-face interviews with people receiving services, and telephone interviews with families, friends, and guardians.  Considerations are things that the individual wants to improve in his or her life and/or things the IM4Q survey team thinks may improve the individual’s life.  Considerations are shared with the Administrative Entity (AE) for appropriate action.  Where immediate health and safety issues are observed, the IM4Q team is instructed to remain with the person receiving services until the situation is addressed.

Information from IM4Q interviews is entered into the Home and Community Services Information System (HCSIS), and when necessary, is used to result in service and support changes in the person’s Individual Support Plan (ISP). In addition, IM4Q data is aggregated to result in AE, provider, regional, and statewide satisfaction, and outcome reports. The aggregate information is used for continuous quality improvement purposes. Systemic quality improvement actions are taken through ODP, AE, and provider quality councils; and reviewed with County MH/DD/ID Advisory Boards.

National Core Indicators (NCI)

National Core Indicators (NCI) is a collaborative effort between the National Association of State Directors of Developmental Disabilities Services (NASDDDS) and the Human Services Research Institute (HSRI). The purpose of the program, which began in 1997, is to support NASDDDS member agencies to gather a standard set of performance and outcome measures that can be used to track their own performance over time, to compare results across states, and to establish national benchmarks.
