Communication and Meetings Structure
Our collaborative meets quarterly. The members listed are also a part of other various coalitions and committees that incorporate the Lifecourse tools in their events.
Goals, next steps or actions plans
Continue working with the education system with transition planning activities to ensure linkage to necessary services and supports and focus on employment and community integration. Continue to implement the initial stages of Charting the Lifecourse at intake and utilizing these tools as needed with complex cases and residential referrals.
In addition, the Regional Collaborative has begun to incorporate/crosswalk Lifecourse tools into the Individual Support Plans.
