Communication and Meetings Structure

Newsletters are now being distributed at least bi-annually if not quarterly.  Information provided contains PA Family Network and SAU1 opportunities, trainings provided by other outside entities, Program updates, IM4Q information, events of interest happening in Cameron/Elk Co., etc.  

Goals, next steps or actions plans

The C/E Family collaborative has now met twice over the past year.  Families that showed at the first meeting suggested a topic to be PA Able Savings Program. The Fall meeting occurred which provided a presentation by PA Able and following discussion among the participants.  Advertising is done through Providers, newsletters, community advertising, etc.  The next meeting will focus more on aging/end of life planning.  These meetings are held in a centralized location for our County Programs and is held in the evening hours, for those that work. 

Transition meetings have begun for FY 23-24 and the AE makes it a priority to participate in every meeting invited to within the area school districts.  During these meetings, the AE discusses the services that are offered to ensure success in their everyday life.  Lifecourse materials are discussed and provided to the individual/family.  Materials include the transition booklet, the One Page Profile/trajectory, along with other materials that not only the individual can complete, but also the family, from their point of view.
