Communication and Meetings Structure
Crawford County AE meets with our local Crawford County SCO (Crawford County Supports Coordination) bi-weekly to review all updates/announcements from ODP including updates from CoP/Lifecourse.
Goals, next steps or actions plans
Crawford County AE providers the Life Trajectory Tool to all incoming intakes to help the individual and their team plan for their future with their SCO. Our goal is to continue to educate our local SCO on all the Lifecourse Tools, so other tools can also be used with the individual, if needed.
AE/SCO/Provider/DHS/Broad Community Partners Represented within Collaborative:
Crawford County AE holds quarterly Quality Council meetings; virtually or in person at CCHS office building where activities around CoP are discussed with local ID/A providers and our community partners such as OVR and Always On Our Own (AOOO)- Our Independent Monitoring for Quality (IM4Q).
CoP AE Lead also attends quarterly meetings for Systems of Care (SOC) as well as Transition Council where AE shares CoP discussion and activities with community partners, local IU5, school districts and OVR.
