The Office of Developmental Programs (ODP) requests your assistance sharing the attached communication, ODPANN 25-057 New Medication Administration (MedAdmin) Agency/Entity/Role Designation/Verification Form and Process. The purpose of this communication is to announce that a new version of the Agency/Entity/Role Designation/Verification form is now available. New and recertifying trainers will be required to have this form filled out by an agency administrator and/or primary contact and will upload the form in the MedAdmin system before beginning the Train-the-Trainer (TtT) course.
