The purpose of this communication is to offer guidance for AEs and SCOs on actions must be taken when an Office of Developmental Programs’(ODP) waiver or base participant is admitted to a NF to ensure a coordinated transition to Long Term Care (LTC) services and prevent service interruptions. The previous announcement instructed submittals of a 1768 to County Administration Offices (CAOs) indicating a transfer of HCBS. This update is being issued to clarify that the 1768 should be a termination of the ODP waiver rather than a transfer. Updates are indicated in red.
